Understanding Achilles Tendinopathy

Pain and tendon changes can develop when the Achilles tendon does not adapt to repeated running load. Exercise selection may differ depending on whether pain is at the heel insertion or higher in the tendon.

Treatment Direction

Adjust Running Intensity

Reduce hills and sprinting, and use comfortable flat running or lower-impact exercise.

Identify Tendon Location

Examination and ultrasound when needed distinguish insertional from mid-portion tendon problems.

Build Calf Strength

Progress heel-raise range and resistance according to the pain location and recovery stage.

Return-to-Running Criteria

Walking and stairs cause stable, manageable symptoms
Repeated single-leg heel raises are possible
Easy flat running does not cause a major next-day pain increase

When to Seek Evaluation Promptly

A sudden pop, inability to raise the heel, or a palpable gap in the tendon requires assessment for an Achilles rupture first.
